ABSTRACT

Background: Treatment of spine bone metastases with stereotactic radio-therapy (SBRT) may produce greater pain relief than palliative radiotherapy. Aim: To evaluate the analgesic response to SBRT. Material and Methods: A search was made in an electronic database of all patients treated by SBRT in our center. We found 20 patients that were treated with SBRT in the spine on 24 sites (lesions). Analgesic response was evaluated at 3 and 6 months after SBRT. Other factors such as age, sex, functional status, diagnosis, metastasis location, dosimetry and toxicity of the treatment were also described. Results: The median follow-up was 8.1 months. Complete pain relief occurred at three months in 74% of the treated sites. At three months, 78% of the patients presented a functional status 0 (ECOG). The median dose used was 24 Gy in 2 fractions. No cases of G3 or greater toxicity were recorded. Conclusions: The analgesic response to SBRT seems to be better than that reported for palliative radiotherapy.

ABSTRACT

Summary Objective: To provide a quantitative profile of the indications and use of stereotactic body radiotherapy (SBRT) in a developing country oncology-based institution. In addition, to describe the patients' and treatment characteristics, and to provide a temporal analysis. Method: SBRT patients treated from 2007 to 2015 were retrospectively evaluated by two independently investigators. Data were stratified and compared in two periods: first experience (FE) (May 2007 to April 2011), and following experience (FollowE) (May 2011 to April 2015). The following parameters were compared between the groups: total number of treated patients and lesions, treatment site, additional image fusion used, formal protocol adoption, and SBRT planning technique. Results: One hundred and seventy-six (176) patients with 191 lesions were treated: 34 (18%) lesions in the FE and 157 (82%) lesions in FollowE. The majority of lesions were metastases (60.3%), and lung (60.2%) was the most common treatment site, followed by spine (31%), and others (8.8%). An average of 1.4 (±0.6) additional imaging exams for delineation was performed. Conformal 3D radiotherapy planning technique was used in 64.4%, and intensity modulated radiotherapy (IMRT) or volumetric-modulated arc therapy (VMAT) in the remaining 35.6% (p=0.0001). Higher rates of curative treatments were observed in FE, as well as more lung lesions, patients ≥ 70 years, 3D conformal, number of additional images and ECOG 0, and all presented p<0.05. The global rate of protocol statement was 79%, lung treatment being the most stated. Conclusion: SBRT application is rapidly increasing in our setting. Treatment sites and planning techniques are becoming more diversified and complex.


Resumo Objetivo: realizar uma análise quantitativa das indicações e do uso de SBRT (stereotactic body radiotherapy) em uma instituição filantrópica. Além disso, descrever temporalmente as características dos pacientes e tratamentos. Método: retrospectivamente, foram coletados, por dois investigadores independentes, os dados de todos os pacientes tratados com SBRT na instituição no período de maio de 2007 a fevereiro de 2015. Dados foram estratificados e comparados em dois períodos: inicial (P1) (maio de 2007 a abril de 2011) e período seguinte (P2) (maio de 2011 a abril de 2015). Os seguintes parâmetros foram comparados entre os grupos: número total de pacientes e lesões tratadas, local de tratamento, uso de imagens adicionais, adoção formal de protocolo e técnica de planejamento. Resultados: foram avaliados 176 pacientes e 191 lesões, sendo 34 (18%) no P1 e 157 (82%) no P2, tratados com SBRT. A maioria das lesões eram metastáticas (60,3%), sendo pulmão o sítio mais prevalente, contabilizando 115/191 (60,2%) lesões, seguida de lesões ósseas (30,8%). Uma média de 1,4 (±0,6) exame de imagem adicional foi usada para o delineamento. Uso formal de protocolo/recomendações foi descrito em 79% das lesões. A técnica 3D-conformada (3DCRT - three-dimensional conformal radiotherapy) foi a mais utilizada para planejamento [123/191 (64,4%) lesões] quando comparada à intensidade modulada e ao arco dinâmico (p=0.0001). O P1 esteve associado a maior número de lesões primárias tratadas (OR 2,09; p=0,0005), lesões pulmonares (OR 3,85; p=0,0198), pacientes ≥ 70 anos (OR 2,77; p=0,0005), uso de planejamento 3D (OR 16; p=0,0001), menor uso de imagens adicionais [0 ou 1 (versus > 1)] (OR 7,5; p=0,0001) e ECOG 0 (vs. > 0) (OR 0,21; p=0,0431). Conclusão: o uso de SBRT aumentou ao longo do tempo na instituição. No P2, notou-se uma evolução técnica, com indicações mais diferenciadas, maior uso de imagens auxiliares para definição do alvo e técnicas de planejamento mais sofisticadas.

ABSTRACT

CONTEXT AND OBJECTIVE: Chordoma is a rare tumor with a high risk of locoregional recurrences. The aim of this study was analyze the long-term results from treating this pathological condition. DESIGN AND SETTING: Cohort study in a single hospital in São Paulo, Brazil. METHODS: This was a retrospective cohort study on 42 patients with chordoma who were treated at Hospital A. C. Camargo between 1980 and 2006. The hospital records were reviewed and a descriptive analysis was performed on the clinical-pathological variables. Survival curves were estimated using the Kaplan-Meier method and these were compared using the log-rank test. RESULTS: Nineteen patients were men and 23 were women. Twenty-five tumors (59.5%) were located in the sacrum, eleven (26.2%) in the skull base and six (14.3%) in the mobile spine. Surgery was performed on 28 patients (66.7%). The resection was considered to have negative margins in 14 cases and positive margins in 14 cases. The five-year overall survival (OS) was 45.4%. For surgical patients, the five-year OS was 64.3% (82.2% for negative margins and 51.9% for positive margins). In the inoperable group, OS was 37.7% at 24 months and 0% at five years. CONCLUSION: Complete resection is related to local control and definitively has a positive impact on long-term survival. .


CONTEXTO E OBJETIVO: Cordoma é um tumor raro e com alto risco de recidiva locorregional. O objetivo deste estudo foi analisar os resultados a longo prazo do tratamento dessa doença. TIPO DE ESTUDO E LOCAL: Estudo de coorte realizado em um único hospital em São Paulo, Brasil. MÉTODOS: Estudo de coorte retrospectivo com 42 pacientes com cordoma tratados de 1980 e 2006 no Hospital A. C. Camargo. Os prontuários foram revistos e foi realizada a análise descritiva das variáveis clínicas e patológicas. As curvas de sobrevida foram estimadas pelo método de Kaplan-Meier e a comparação entre elas, pelo teste de log-rank. RESULTADOS: Dezenove pacientes eram homens e 23, mulheres. Vinte e cinco tumores (59,5%) estavam localizados no sacro, 11 (26,2%) na base do crânio e 6 (14,3%), na coluna móvel. A cirurgia foi realizada em 28 pacientes (66,7%). A ressecção foi considerada como tendo margens negativas em 14 casos e margens comprometidas em 14 pacientes. A sobrevida global (SG) em 5 anos foi de 45,4%. Para os pacientes cirúrgicos, a SG em 5 anos foi de 64,3% (82,2% para as margens negativas e 51,9% de margens positivas). No grupo inoperável, a SG em 24 meses foi de 37,7% e 0% em 5 anos. CONCLUSÃO: A ressecção completa está relacionada com o controle local e, definitivamente, tem impacto positivo na sobrevida a longo prazo. .

Dysembryoplastic neuroepithelial tumors (DNETs) arise mostly in the supratentorial cerebral cortex. A very rare case of intraventricular DNET with diffuse ependymal involvement, which causes spinal drop metastasis, is presented.

ABSTRACT

As metástases ósseas que acometem o segmento torácico e lombar da coluna vertrebal têm como principais complicações o fenômeno doloroso e a compressão medular ou radicular. O tratamento adotado dependerá das condições clínicas e neurológicas do paciente e do grau de invasão tumoral. Foram analisados, prospectivamente, 32 pacientes acometidos por metástases do segmento torácico e lombar da coluna vertebral, sendo tratados com cirurgia de descompressão e estabilização interna seguida por radioterapia ou irradiação com mobilização externa. A seleção dos grupos foi estabelecida de acordo com a sensibilidade do tumor a radioterapia, condições clínicas, grau de estabilidade da coluna, grau de compressão medular/radicular e opção do paciente. A escala de Frankel e o teste visual de dor foram aplicados no momento do diagnóstico e após 1 e 6 meses. O grupo cirúrgico apresentou melhores resultados, mantendo a deambulação por mais tempo, pois manteve a força muscular nos pacientes Frankel E, propiciou a reversão do déficit neurológico pré-operatório em 61,5 por cento dos casos e propiciou melhora significativa da dor.


Bone metastases at the thoracic and lumbar segment of the spine are usually presented with painful sensation and medullar compression. The treatment is based on the clinical and neurological conditions of the patient and the degree of tumor invasion. In the present study, 32 patients with spinal metastasis of thoracic and lumbar segment were prospectively analyzed. These patients were treated by decompression and internal stabilization followed by radiotherapy or irradiation with external immobilization. The election of the groups was in accordance with the tumor radiotherapy sensitivity, clinical conditions, spinal stability, medullar or nerve compression and patient's decision. The Frankel scale and pain visual test were applied at the moment of diagnosis and after 1 and 6 months. The surgical group had better results with preserving the ambulation longer and significant reduction of pain.

ABSTRACT

Our study is about 71 cases of the spine tumors treated in the neurosugical department during 10 years [82-92]. A male predominance has been noticed [59%], and the average age was 43 years. Ninty three percent of the patients has been examinated before the first year. The symptomatology that prevailed at the beginning of the disease was the pain. The dorsal spine was the most affected [45%]. The radiologie exams [radiography, myelography and CT Scann] grave a quite exact idea about the lesions. Fifty six patients [80%] was operated. The surgery consisted of a partial or total removal of the tumor in 13 cases and of a laminectomly in the other cases. Pathology showed a prevailing of the metastatic lesion [60%]. The evolution was characterised by an improvement 19,7%, no change state in 56%, an aggravation in 15% and a death in 7%

ABSTRACT

La terapéutica de los pacientes con metástasis en la columna vertebral es aún controversial en los centros especializados, debido quizás a la falta de sistematización de sus variables clínicas. El objetivo del estudio fue demostrar que mediante el uso de diferentes escalas clínicas, se estratifican las metástasis óseas, y así seleccionamos un grupo de pacientes con lesiones vertebrales intracompartamentales que fueron tratados con cirugía "agresiva" y radioterapia adyuvante. Se estudiaron de manera prospectiva 12 pacientes con lesiones metastásicas en una vértebra a quienes se les realizó tratamiento quirúrgico del tipo de resección marginal e irradiación con 200 cGy; el criterio de inclusión fue que tuvieran más de 3 meses de expectativa de vida, que la lesión destructiva se localizara en la vértebra con límites intracompartamentales, o incipientemente extracompartamentales (Mos 3), que presentaran alteraciones en la estabilidad biomecánica de la columna y/o compresión neurológica, o que habiendo sido tratados con radiaciones no hubieran mejorado. Todos los pacientes tratados presentaron mejoría del dolor (1 a 2 unidades en la escala de la OMS) y se restableció la estabilidad mecánica de la columna; en 5 pacientes hubo mejoría neurológica con disminución de 1 a 2 niveles en la escala de Frankel, y en 5 pacientes hubo aumento de 10 a 20 unidades en la escala de Karnofsky; 7/10 pacientes tuvieron sobrevida de 8 a 18 meses con una mejor calidad de ésta. Concluimos que en pacientes seleccionados con metástasis vertebrales Mos 3 que tengan expectativa de vida de más de 3 meses, el tratamiento con cirugía ®agresiva¼ más radioterapia postoperatoria, ofrece una alternativa importante para mejorar su calidad de vida
